The medical certificate (certificado médico) is one of the more confusing NLV renewal NLV requirements because it is not consistently required by every Oficina de Extranjería. Some provinces always ask for it; others have not required it for years. Understanding when you need it — and what it must say — could save you significant hassle.
When Is a Medical Certificate Required for Renewal?
The medical certificate is a standard requirement for the initial NLV application at the Spanish consulate in your home country. For renewals inside Spain, its requirement varies:
- First renewal (year 1 → years 1–3): Many provinces do request it, particularly for applicants over 65
- Second renewal (years 3 → 5): Less commonly required, but some provinces still ask
- Your specific province: Check with local expat communities or your immigration specialist — requirements vary considerably between Andalusia, Madrid, Catalonia, and other regions

What the Medical Certificate Must State
The medical certificate for NLV purposes is a specific document — not a standard GP letter or health check result. It must confirm that you:
- Do not have any disease that could pose a risk to public health according to international health regulations
- Do not have any disease listed in the 2005 International Health Regulations
This phrasing is important. The certificate should specifically reference the International Health Regulations (Reglamento Sanitario Internacional 2005) or state "no padece ninguna de las enfermedades que pueden tener repercusiones de salud pública de conformidad con el Reglamento Sanitario Internacional." If your doctor produces a generic health certificate without this specific reference, it may be rejected.
Getting a Medical Certificate in Spain
If you are already resident in Spain and need a medical certificate for renewal, you have several options:
- Your Spanish GP (médico de cabecera) — if you are registered with the Spanish public health system, your GP can issue the certificate. Explain you need it for residency renewal purposes and provide the specific wording required.
- A private clinic — many private doctors in Spain are experienced in producing NLV medical certificates and know the exact format required. A private appointment typically costs €30–80 and can often be done quickly.
- Some pharmacies — a small number of pharmacies in Spain can refer you to a doctor for a quick certificate appointment
The certificate should be in Spanish or accompanied by an official translation. It must be signed by a registered doctor and, in some cases, countersigned or stamped by the local health authority.
Getting a Medical Certificate from Your Home Country
If you need a medical certificate from your home country (for example, if you are applying for your renewal from outside Spain, which is unusual but possible), the process is:
- Obtain the certificate from a registered medical practitioner in your home country
- Have the certificate apostilled by the appropriate authority
- Obtain an official Spanish translation (traducción jurada)
UK applicants can obtain apostilles from the Foreign, Commonwealth & Development Office (FCDO). US applicants use state-level authorities depending on the state the document originates from.
Validity Period
Medical certificates are generally accepted if they are no more than 3 months old at the time of your renewal appointment. Do not obtain your certificate months in advance — time it to be current when you attend your Oficina de Extranjería appointment.
What Happens if Your Province Does Not Require It
If you prepare a medical certificate and your province does not require it, there is no problem — simply do not include it in your document pack unless asked. Having it available as a supplementary document is always a safe position.
If you discover after your appointment that a medical certificate was expected but you did not bring one, you may receive a requerimiento (formal request) from the office giving you 10 days to submit the missing document.
